Superbill Receipts are a Critical Component of US-Based Lactation Consultant Practices

Separate Parent and Child Versions

LactForms superbills are separate parent and child versions, available both as a package and individually for those who only see parents or children. (A combined superbill version is no longer available now that most insurance companies require separate superbills for each patient—parent and child—since billing codes only apply to one person.)
