What's a PDF?
"PDF" is an acronym forĀ Portable Document Format, created by the Adobe software company in 1993 as a standardized way to "lock in" document style and content so it doesn't change when viewed on various devices and platforms, software, hardware, or operating systems. It was released as an open standard in 2008 and is now maintained by the International Organization for Standardization (ISO).
LactForms PDFs
LactForms use the PDF document format so they're easy to use and share without changing style, format, or layout. LactForms PDFs are designed for easy use electronically (fillable format) or as printed out as a hardcopy (printable format). They include headers at the top of each form that can be personalized on a computer, tablet, or smartphone using any PDF reader app/program or browsers with the free Adobe Acrobat Reader plugin (extension). For best results, use theĀ free Adobe Acrobat Reader app/program.
Fillable versions can be emailed to the client/patient to fill out before the consultation. Printable versions can be printed out and given to them during the consultation.
