Editing and Styling Your Header
On LactForms, the header is the designated section at the top of each form that provides the Lactation Consultant’s contact and company information required by the client/patient’s insurance company for reimbursement of fees paid to you for your services and/or products provided. All LactForms include one of four headers:
- HeaderBox (image only)
- HeaderCenter (centered professional name and information)
- HeaderCompany (with your company name)
- HeaderLogo (with your logo)
They can be changed at no cost upon request and a custom header can be designed for a nominal fee. Detailed header information here.
LactForms headers use fillable fields to add text or an image with your professional information, which can be styled in the free Adobe Acrobat Reader app/program on PC/Windows and Mac (but not on mobile devices). The form can then be saved with a different filename to create a template with your personalized header for ongoing use.
Depending on your PDF program and/or your settings, you may not see highlighting—transparent color shading—on the fillable areas, but they ARE fillable when the fields are selected. The fillable fields highlights will not show on a hardcopy printed page.
Editing Your Header
Enter your information into each of the header’s fillable fields:
1. Select the text field:
2. Highlight the default text:
3. Delete the highlighted default text:
4. Enter your information and hit enter:
5. Style the text as shown in next section (styling is only available on PC/Windows and Mac, but not mobile devices).
6. Save with a different filename to create a template for ongoing use.

When entering text into a fillable field, if your computer or device “dings” (makes an error sound) or a plus sign in a black box appears on the right side of the field, it means there is more text than allowed, so just delete or abbreviate some text, or change the font size as shown below |  |
Styling Your Header
After entering your information into the header’s fillable fields, the text styles can be modified with the free Adobe Reader app/program on PC/Windows and Mac computers, but not on mobile devices (tablets and phones).
1. Select the text field:
2. Highlight the text:
3. Enter Ctrl+E (PC/Windows) or Cmd+E (Mac) to open the Form Field Text Properties toolbar:
4. Change the color, font, size, etc, to match your branding:
5. Select the More button for additional options:
6. The More button opens another window with extra options—changes appear in real-time as they’re modified:
7. After changing the text style, hit enter or select another field.
8. When all header fields have been edited and styled, save the form with a new filename to create a template for ongoing use.
