Fillable and Printable Formats

All LactForms are available in both Fillable format for digital use and Printable format for hardcopy use. The Fillable option includes both the Smart Fillable format for use on computers and laptops (both PC and Mac) and the Mobile Fillable format for use on mobile devices (both iOS and Android).  The Printable option includes both the standard printable format and the printable format with printer's marks. 

Five Header Options

All LactForms PDF documents are available with five header options at the same cost to meet your practice style.  And there's no charge to switch one for another.

Multiple Color Options

LactForms Superbills Version 8.x are available in six colors (blue, green, grey, orange, pink, and purple).  The Referral Script forms are available in blue and grey, with more colors on the way. The Charting forms will be upgraded with new color designs when the new version is released in early 2025.  All forms can be converted to custom colors to match your branding for a nominal fee.

More Languages Coming Soon

After many requests for LactForms translations into other languages for expanded use around the world, LCs fluent in other languages are needed as translators since it takes an understanding of lactation terminology. In return, they'll receive a complimentary complete set of LactForms in their preferred format and color, including a custom header.  There's been a great initial response to this call for translations, with these already underway:
