Fillable format LactForms are digital PDF files with fillable and auto-calculating fields designed for easy electronic (digital) use.  Perfect for paperless offices, linking on your website, and emailing to clients/patients to fill out electronically.  Very user-friendly—after entering the required information, the PDF file can be saved with a new filename to preserve the entries, emailed to the client/patient, or printed out and given to them during the consultation.  Comprehensive detailed instructions are included.

Fillable LactForms are available in two formats, Smart Fillable and Mobile Fillable.  The Smart Fillable format has more complex functionality to ensure forms are completed correctly.  The Mobile Fillable format has fewer bells and whistles so it works on the more limited PDF apps for mobile devices.  Both formats are included with each LactForms fillable form purchase.
