Format Options

LactForms are available in two main format options, Fillable format for electronic use and Printable format for hardcopy printing.  Version upgrades are always free and sent automatically (reach out if you need the newest version).

Fillable Formats

Fillable format LactForms are digital PDF files with fillable and auto-calculating fields designed for easy electronic (digital) use.  Perfect for paperless offices, linking on your website, and emailing to clients/patients to fill out electronically.  Very user-friendly—after entering the required information, the PDF file can be saved with a new filename to preserve the entries, emailed to the client/patient, or printed out and given to them during the consultation.  Comprehensive detailed instructions are included.

Fillable LactForms are available in two formats, Smart Fillable and Mobile Fillable.  The Smart Fillable format has more complex functionality to ensure forms are completed correctly.  The Mobile Fillable format has fewer bells and whistles so it works on the more limited PDF apps for mobile devices.  Both formats are included with each order for workplace flexibility.

Printable format LactForms are digital PDF files designed for easy printing as hardcopy documents and completing (writing) on paper. Comprehensive detailed instructions are included.  

Printable LactForms are available in two formats, Printable and Printable with Printer Marks formats.  Both formats are included with each Printable form purchase.

Ready for printing in your office or professional printing in bulk

Ideal for professional printing on three-part carbonless forms (other companies that provide this service online can be found searching on “carbonless forms” or “NCR forms”)

Easy to email the PDF file to clients (patients) for them to print and fill out before the appointment or you can print them yourself and provide a copy in person 

Copyright release (required by many professional printers) included

Printable with Printer's Marks

Same benefits as the Printable version
