The pricing depends on whether this color combination will be exclusive(only for you) or non-exclusive(added to LactForms color options for sale to other customers), and how many LactForms will be converted (forms sold separately).
This LactForms Custom Colors Service must be purchased for each form type to be converted (Superbills, Clinical Charting, Referral Script, Rentals).
Converted forms will be sent by email within one week after the form is submitted (if longer is needed, you'll be contacted by email).
